VACCINATION RECORD (MINOR CHILDREN)
IDENTITY CARDS, OR PASSPORTS AND RESIDENCE PERMITS WHERE APPLICANTS ARE FOREIGN NATIONALS
IF THERE ARE ANY HEALTH ISSUES, THEN A RECENT MEDICAL DIAGNOSIS IS REQUIRED

Applications for assessment are accepted by the Social Service Department of the Association, following a telephone call
to 210 721590 (Monday to Friday 10:00-13:00)
